The transfer problem

Delivering training and changing practice are not the same thing

Clinical training has a transfer problem. The gap between what clinicians learn and what they do in practice is one of the most persistent challenges in clinical education, and research consistently shows that delivering training does not reliably change how people work.

The roots usually lie in design. Training that is dense, compliance-led, or built for everyone at once tends to stay theoretical. That is the problem I exist to solve. I design training for transfer, so that what staff learn holds up at the bedside.

My mission

The purpose behind my work

My mission is to close the gap between what clinicians learn and what they do in practice. Too much healthcare training is completed and forgotten, never reaching the bedside where it matters. I design training that transfers, so that learning holds up in the moments that count, for clinicians and their patients.

That work has a second purpose. We are fortunate in metropolitan Australia to have a robust healthcare system, something rural and developing regions are not always afforded. So I run BM Learning Design as a profit-for-purpose social enterprise, directing 20% of earnings to fund free health education in these communities, partnering with not-for-profits to reach clinicians and patients where the need is greatest. Every project you commission helps extend good training further, in its reach as well as its quality.

Meet Bec

I founded BM Learning Design after a decade in healthcare. I spent seven years as a critical care nurse, serving alongside this in the Army Reserves, before moving into education as a nursing lecturer at Australian Catholic University. Today I work as an instructional designer, designing evidence-based learning for clinicians every day.

Like every clinician, I've sat through the thirty-minute compliance module I never thought about again. I've also taken part in thoughtful training that landed well on the day, then quietly faded because nothing reinforced it back on the ward. And, occasionally, I've done the rare course that stayed with me and changed how I practised. These experiences drove me to study what makes learning stick. The difference between training people forget and training that lasts is almost never effort or intelligence. It's design.

My perspective bridges clinical practice and education, backed by three postgraduate qualifications: a Graduate Diploma in Nursing Science (Intensive Care Nursing), a Graduate Certificate in Clinical Education, and a Master of Education specialising in Digital Technologies, which I'm currently completing.

I design for the clinician who has been let down before, the one whose time was wasted on something that never reached their practice. Because when training doesn't transfer, it doesn't just cost a few hours. It costs clinicians in their work, and ultimately the patients in their care.

Know what to build

Training Needs Analysis

I understand the problem before building the fix. I analyse the performance gap, your learners, and the clinical context to define what training actually needs to do. You get a report that explains what training to build and why.

Most eLearning companies will happily take your money to make a course without asking whether training is the right answer. I won't. Before you invest a cent in a build, I make sure training is genuinely the solution to your problem.

Training that can't be measured can't be improved. Using Kirkpatrick's model as a foundation, I help you evaluate all four levels, reaction, learning, behaviour, and results, so you know whether training is actually driving the change you invested in.

How I did it: I gave the workshop toolkit to an AI assistant and worked with it to draft a storyboard, screens, on-screen text, interactions, and knowledge checks. I then refined the storyboard with my learning-design judgement and used AI again to build it out as a SCORM eLearning package. What took days now takes hours, and the same approach works to reformat training materials into job aids, bedside cards, and more.

Write learning outcomes that set the target for transfer

A good outcome describes what your learner will do in practice, not just what they will know. Fill in the four parts below, and a clear, measurable outcome will build as you go.

1Who is the learner?
Name the specific role. "Graduate nurses" works better than "staff", because it keeps the outcome grounded in a real person.
2What will they be able to do?
Choose the level of thinking, pick an observable verb, then describe the task. Try to avoid "understand" or "know", since you cannot see either one happen.

3Under what conditions?
The real circumstances they will work in. This is what makes an outcome clinical rather than classroom. Optional, but worth adding.
4To what standard?
How well, how fast, or against what benchmark. This is how you will know the outcome has been met. Optional.

Worked example · a full lesson

Peripheral intravenous cannulation

A short lesson usually needs only one to three outcomes. Here is how a set might look, moving from understanding through to judgement.

By the end of this training, registered nurses will be able to:

Understand Explain the indications, contraindications, and common complications of peripheral intravenous cannulation.
Apply Demonstrate insertion of a peripheral intravenous cannula in a clinical setting, according to hospital protocol.
Evaluate Determine when to escalate a difficult or failed cannulation to a senior clinician.

Notice each outcome opens with an observable verb, and together they build from knowing to doing to judging.
